We are looking for a self-motivated, enthusiastic and committed band 6 Nurse to work as our Senior Triage Nurse in the Award Winning E&N Referral Hub based in Robertson House, Stevenage.

The successful nurse will provide expert clinical triage of complex nursing referrals in our E&N Referral Hub alongside their therapy and social worker colleagues who rotate into the Referral Hub. The role includes telephone triaging patients to establish clinical needs and direct patients to the appropriate locality team or service. You will also be providing expert nursing advice, support and information to patients and colleagues on social prescribing and self-management as well as signposting to other services.

The successful applicant will be part of an energetic, fast paced, patient focused team triaging patients for community nursing while working collaboratively with GPs, Acute hospitals, Social Care, Hospices and other third sector organisations. You will work closely with the Referral Hub Clinical Manager in the hub and have opportunity to triage patients with a wide range of clinical presentations referred for community nursing.

The Referral Hub is open 8.00am – 6.00pm, the successful candidate will be required to join our triage rota covering these hours. Weekend working will feature.
